About Us

Zócalo Health is a tech-enabled, community-oriented primary care organization serving people who have historically been underserved by the one-size-fits-all healthcare system. We partner with health plans, providers, and community organizations to deliver culturally competent primary care, behavioral health, and social care.

Our model is built for populations with high medical and social complexity, where fragmented care drives poor outcomes and unnecessary cost. We combine local, community-based teams with virtual care and modern technology to deliver coordinated, whole-person care where members live and receive support.

Founded in 2021, Zócalo Health is backed by leading healthcare and mission-aligned investors and is scaling rapidly across states and populations. We are building a durable care platform designed to perform in constrained healthcare environments and to lead the shift toward accountable, value-based care.

Role Description

The Training Associate will join Zócalo Health during a period of rapid growth and increasing operational complexity. This role exists to help scale and execute a community-oriented primary care model that delivers measurable outcomes for high-need members and health plan partners.

As part of a fast-growing team, you will be responsible for leading the design, development, and delivering training programs specifically for Community Health Workers (CHWs). You will support CHWs in building the skills needed to engage members, navigate community resources, and drive improved health outcomes, while ensuring training is culturally responsive, practical, and grounded in real-world workflows.

This position reports to the Learning and Development Manager.

The Training Associatewill contribute in the following ways:

  • Own the design and facilitation of the CHW onboarding and ongoing training focused on member engagement, care coordination, documentation, and community resource navigation grounded in best practices and industry standards
  • Create and maintain training materials (scripts, role plays, workflows, and field guides) that reflect real CHW scenarios and support consistency across the team
  • Partner with CHW managers and field teams to identify gaps, gather feedback, and continuously improve training based on member needs, program goals, and performance data
  • Work closely with the Learning and Development manager to maintain an updated Learning Management System (LMS) and track training completion across the CHW team
  • Develop cheat sheets and other supporting materials that can be used as a reference to support the Community Health Workers day-to-day operations

Qualifications

  • 4-5+ years of experience in training, education, or enablement, ideally supporting frontline or community-based teams
  • Experience working with or closely alongside Community Health Workers, patient navigators, or similar roles
  • Strong facilitation skills with the ability to make training interactive, practical, and accessible for diverse learning styles
  • Experience in Medicaid/Medi-Cal, Enhanced Care Management (ECM), or community-based care models

Preferred Qualifications

  • Bilingual (Spanish/English), with the ability to design, deliver, and adapt training materials effectively in both languages
  • Proven experience developing engaging training content, including role plays, motivational interviewing exercises, and practical, field-based learning tools
  • Strong foundation in adult learning principles and instructional design models, with the ability to tailor content to diverse learner needs
