Requirements
- Establishes professional relationships with clients
- Reviews documentation and pre-screens clients for non-shelter alternatives
- Conducts intake and psycho-social assessments of clients' and clients' families' needs
- Prepares initial and periodic revisions of independent living plans
- Assists clients in attaining their goals by referring them to permanent housing opportunities and community resources
- Monitors clients' progress toward their goals via regularly scheduled contact and document via progress notes
- Acts as client liaison/client advocate with outside organizations
