We are a passionate team of physicians, technologists, and health plan leaders who recognize the critical role of PCPs, especially highlighted during the pandemic, and the under-resourced nature of their practices. We started with Traditional Medicare, as it represents the largest undisrupted healthcare market in the U.S., and we leverage models like ACO REACH to help primary care practices thrive. Our technology platform is designed to be customizable to the unique needs of each practice, specifically for success in value-based care models. We provide stable payments to offer peace of mind and revenue stability, and participating providers can earn additional revenue through the successful management of their patient population. By integrating with technology and focusing on proactive care, we help reduce overall healthcare costs and improve performance in value-based arrangements.
