In 2014, MedZed was founded with a clear purpose: to bridge the gap for the most vulnerable members of our healthcare system - those who had fallen through the cracks of traditional outreach and care management. The founders recognized that high-risk Medicaid and Medicare members often cycled in and out of emergency rooms, lacked connections to primary care, and faced overwhelming social determinants of health that prevented them from accessing the help they needed.

Today, MedZed delivers on that original vision through a powerful combination of compassionate, locally-based Community Health Navigators and proprietary technology. The company's innovative model reaches members wherever they are - often in their own homes - and builds trust through persistent, empathetic engagement. By addressing both clinical needs and social barriers like housing instability, food insecurity, and lack of insurance coverage, MedZed successfully re-integrates disengaged members into primary care while dramatically reducing costly emergency department visits and hospitalizations.

MedZed partners with Medicaid Managed Care Plans, Medicare Advantage Plans, and risk-bearing provider organizations across 11 states to deliver Enhanced Care Management and Community Supports services. With 22 health plan partners and over 50,000 high-risk individuals served, the company has documented a 40% reduction in both ED visits and inpatient admissions per thousand members - a measurable impact that proves when healthcare meets people where they are, outcomes improve for everyone.
