This key position will be based out of the employee’s home office, which will need to be located within the state of Oregon.
The GOBHI Client Care Coordinator will work with members and community partners to ensure coordination of care for behavioral health needs. The Client Care Coordinator is a key position with a primary purpose to make sure members’ needs are met by developing plans of care. This position works directly with members to assist with barriers to accessing behavioral health care as well as help identify various social determinants of health that a member needs assistance with and help connect that member with appropriate resources. This position will also support members with educational materials and resources, including some behavioral health skills. This position will also serve as support to the clinical care coordinators (QMHPs) at the direction of their supervisor; this may include assisting with care management duties as needed.

- Document and complete assessments which include strengths and potential needs, life skills, physical mental health and substance use histories, and social support needs.
- Develop individualized plans of care.
- Teach and provide telephonic support for members and those around them.
- Coordinate with providers as needed.
- Coordinate with member and members teams as needed.
- Document and track progress towards goals.
- Facilitate communication between members and their support systems.
- Participate in team meetings associated with member care.
- Strong knowledge of resources in reach region.
- Connects members with behavioral health resources by coordinating care between primary care, emergency departments and behavioral health providers.
- Be available to members and community partners by responding to requests and inquiries.
- Connect members to other community resources including housing, employment, nutrition, etc.
- Assists GOBHI in tracking member outcomes.
- Drives to meetings or other GOBHI locations as needed.
- Other duties as assigned.
Requirements
What is needed to succeed in this role:
- Two years’ experience in healthcare administration or case management.
- Experience providing behavioral health skills training and psychoeducation.
- Ability to communicate effectively at multiple levels.
- Effectively work with a variety of community stakeholders.
- Strong planning and organizational skills.
- Technology savvy – comfortable using MS and Google Suite software products.
- Maintains good driving record and required vehicle insurance.
- Education – Bachelor’s degree in health/human services field or equivalent years of experience required.
- Qualified Mental Health Associate (QMHA) certification required.
