Primary Care Physician (Full-time Remote, Telehealth)

We’re looking for a Board Certified (or Eligible) Primary Care Physician to join our team and deliver high-quality, preventive-focused care through telehealth.

What You’ll Do

  • Provide routine telehealth visits with Medicare patients (E/M and Chronic Care Management)
  • Create and manage personalized care plans for ongoing conditions
  • Collaborate with our integrated care team (nurse case managers, NPs, CHWs)
  • Ensure thorough documentation in our EHR and compliance with quality measures
  • Engage patients in preventive care to reduce avoidable complications

What We’re Looking For

  • MD or DO, Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics
  • Active medical license in Georgia (or Missouri) — multi-state licensure a plus
  • Credentialed with major plans: UHC, Aetna, Humana
  • Strong interest in preventive care, senior health, or chronic condition management
  • Tech-savvy and comfortable practicing via telehealth
  • Collaborative, mission-driven mindset with excellent communication skills
