Position Overview

The Family Facilitator, part of the Pediatric Complex Care Resource Centers (PCCRC), employs a blend of expertise and contract compliance to support children with developmental disabilities. Utilizing strong project management, critical thinking, decision-making, and communication skills, the Family Facilitator champions family life for children currently residing in institutions or facing hospitalization with the risk of institutionalization.

The role involves initiating contact with families of children with complex medical needs, introducing the concept of caring for their child in a home setting, and guiding the transition process. This encompasses facilitating families' efforts to bring their children home or opt for another suitable home support plan. The position entails regional travel, potentially overnight stays, and occasional evening and weekend commitments.

**Must reside in southwest quadrant of Pennsylvania.

Job Responsibilities

  • Identify children suitable for discharge from hospitals or congregate care settings. Conduct visits, review records, and engage with families to gain insight into their specific needs.
  • Educate families on available in-home services and explore options for care in a family-based setting.
  • Forge close partnerships with local authorities, managed care entities, provider agencies, community organizations, and stakeholders. Provide consultation, education, and technical assistance to families as needed.
  • Collaborate closely with hospital discharge planning teams and supports coordinators to ensure a seamless transition process.
  • Address barriers to discharge by identifying and facilitating solutions. Deliver presentations to families and team members and coordinate any necessary environmental or home modifications.
  • Oversee the transition to the Pediatric Coach role and fulfill pediatric coach duties until a smooth transition is achieved.
  • Gather and maintain data, generating reports for stakeholders. Remain updated on current literature, regulations, and research related to long-term care, mental illness, medical conditions, and geriatrics.
  • Attend local, regional, and statewide project meetings to stay informed and contribute insights.

Required Qualifications, Skills, Knowledge and Experience

  • Bachelor’s degree in Social Work, Psychology, Education, Special Education, or a related field with a focus on developmental disabilities.
  • Possession of appropriate state licenses as mandated by the Pennsylvania Licensing System for both education and profession.
  • Minimum of 3 years of demonstrated experience working with individuals, particularly children that are diagnosed and coping with complex medical needs.
  • Possession of a valid PA driver’s license and insurance for personal vehicle transportation.
  • Compliance with clearances mandated by the Pennsylvania Child Protective Services Law.
  • Demonstrated experience of 3 years in working with individuals, particularly children, coping with developmental disabilities and autism.
  • Proven track record in project development and coordination, with experience collaborating across multiple human service entities catering to children and families.
  • Desired qualifications include bilingual language proficiency and prior experience in case management.
  • Comprehensive understanding of public healthcare and community organizations pertinent to children with developmental disabilities, encompassing knowledge of physical and mental health complexities, service system architecture, and insurance company regulations and resources.
  • Strong facilitation, customer service, and conflict resolution abilities.

Compensation

The compensation for this role is $62,900 annual salary. Based on our compensation program, an applicant’s position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
