What I'm looking for

I’m looking to provide high-impact, patient-centered case management—using telehealth and clinical expertise to manage complex chronic conditions, educate patients, and partner with care teams to improve outcomes.

I’m a Registered Nurse and telehealth case manager who delivers remote and in-home nursing care for complex acute and chronic conditions. In my Penn Medicine at Home roles, I performed remote patient monitoring, completed virtual patient assessments, triaged needs, and provided patient education while collaborating in real time with home nurse, therapy, and CNA staff.

I’ve built my practice around strong clinical documentation, medication reconciliation, and coordination with primary care, specialty, and hospital physicians to escalate complex cases as needed. Earlier in my career, I provided total nursing care in rehabilitation (traumatic brain injury, orthopaedic, and medically complex patients), including phlebotomy, infusions, wound care, catheter and drain management, and admissions/discharges—while developing and implementing patient goals and plans of care.
