In my current role as a Licensed Vocational Nurse, Lead LVN Case Manager at Angel Care Hospice and Palliative, I coordinate patient intake, ensure orders are received and processed timely, and support multiple care aspects. I participate in hospice IDT meetings and maintain accurate patient care records while providing caring interactions that promote patient/family satisfaction.

Previously, at City of Hope, I worked in Patient Referral Services using Epic and Salesforce to complete clinical intake and ensure patients were scheduled with the correct provider and service line. I also bring telehealth and EMR experience from earlier clinic work, including training staff and providers, supporting a transition from Allscripts to Epic, completing digital retinal images/exams, administering injections and treatments per provider orders, and documenting vitals and patient history in the EMR.
