Raquill Brooks

I’m a dedicated and compassionate Social Pedagogical Worker (SPW Level 4) with over three years of experience supporting individuals with physical and intellectual disabilities. I develop individualized care plans, set measurable goals, and monitor progress through ongoing observation and documentation.

In my role, I provide daily coaching and guidance that promotes independence, healthy routines, and personal development. I support Activities of Daily Living (ADL), educate clients using individualized teaching methods, and use motivational support and behavior change strategies to help clients build confidence and long-term progress.

I collaborate closely with physicians, therapists, psychologists, and multidisciplinary healthcare teams, including families, to ensure continuity of care. I’m now seeking to transition into a Remote Health Coach position where I can use my coaching, communication, and care coordination experience to help individuals achieve healthier lifestyles and long-term wellness goals.
