At Postgraduate Center for Mental Health, I provide community-based mental health and substance use services as part of an Assertive Community Treatment team in Queens. My work includes crisis intervention, harm reduction support, motivational interviewing, and relapse prevention counseling.
At Essen Health Care, I coordinated care for Medicaid members across interdisciplinary teams, supported transitions after emergency department visits and hospital discharges, and completed health assessments. I documented care using eClinicalWorks, Foothold Care Management, and ePACES.
As an Environmental Research Fellow at AIRnyc, I co-developed a project focused on reducing asthma-related emergency visits through data-informed environmental interventions. I built an interactive dashboard combining patient health data with environmental metrics to support underserved NYC communities.
At NYC Preparedness and Recovery Institute, Columbia University, I developed an integrated mobile app concept for care coordination, caregiver support, and emergency preparedness. My background also includes dental public health outreach in India, including tobacco cessation programs and dental camps.
