Hope Fields-Brown

With 26 years of diverse nursing experience, I have worked across various specialties including disaster nursing, travel nursing, school nursing, and case management. My extensive background allows me to provide comprehensive care to both pediatric and adult populations. I am licensed in North Carolina and New York, and I hold a certification as a school nurse, which enhances my ability to address the unique needs of students and their families.

Currently, I serve as a Case Manager for AmeriHealth Caritas Insurance, where I coordinate patient care and connect members with appropriate community resources. My role involves health education and ensuring timely access to services, which is crucial for addressing the social determinants of health. My goal is to empower members to achieve optimal health and effective self-care management.

In addition to my clinical experience, I hold a Doctor of Nursing Practice (DNP) and a Bachelor of Science in Nursing. My education has equipped me with the skills necessary to excel in nursing leadership and advocacy, further enhancing my commitment to improving patient outcomes.
