Role Overview
We are seeking a knowledgeable and compassionate Diabetes Educator to join our team, focused on empowering individuals with diabetes to manage their health effectively. In this role, you will utilize your expertise to provide education, support, and resources to patients, helping them navigate the complexities of diabetes management.
Responsibilities
- Conduct comprehensive assessments of patients' diabetes management needs and tailor educational plans accordingly
- Deliver one-on-one and group education sessions on diabetes self-management, including nutrition, exercise, medication management, and blood glucose monitoring
- Collaborate with healthcare providers to develop and implement individualized care plans
- Provide ongoing support and motivation to patients, addressing their concerns and helping to overcome barriers to adherence
- Utilize technology, including telehealth options, to reach and educate patients effectively
- Stay current on diabetes research and best practices to ensure the delivery of evidence-based education
Required and Preferred Qualifications
Required:
- Registered Nurse (RN), Registered Dietitian (RD), or comparable healthcare professional with certification in diabetes education (e.g., CDCES)
- Minimum of 2 years of experience in diabetes management or education
- Strong knowledge of diabetes care standards, treatment options, and management strategies
- Exceptional communication and interpersonal skills, with the ability to engage and motivate patients
Preferred:
- Experience in a clinical setting, particularly in endocrinology or diabetes care
- Familiarity with health technology tools and remote patient monitoring systems
Technical Skills and Relevant Technologies
- Proficiency in electronic health record (EHR) systems
- Experience with diabetes management software and mobile applications
- Ability to analyze patient data and adjust education plans based on outcomes
