Join a groundbreaking clinical team of MDs, mid-levels, registered nurses (RN), social workers, health coaches, nutritionists, Certified Diabetes Care and Education Specialists (CDCES) and others delivering virtual pediatric type 1 diabetes care across the nation. Verily’s Onduo is a virtual, precision care management solution that empowers patients with chronic conditions to lead healthier lives.
As a Population Health Manager you’ll work as part of an inter-professional team to help pediatric members with type 1 diabetes, and their caregivers, manage their conditions. Population Health Managers collaborate with Onduo physicians and other members of the care team to create and execute evidence based protocols and care plans for our members. You’ll evaluate member care plans and progress, identifying and helping people overcome risks, barriers and behaviors which interfere with obtaining maximum benefits from medical care, with an ultimate goal of supporting self-management skills and achieving positive health outcomes.
Responsibilities
- Provide clinical care management services across the continuum of care to members and their caregivers.
- In collaboration with physicians and specialists, develop and execute specialist level interventions based on Onduo’s clinical protocols. This includes, but is not limited to cases requiring deeper clinical assessment or education, medication review and optimization, or any other timely clinical care.
- Work with other care team members to execute on the clinical care plan to maximize healthcare outcomes and facilitate wellness.
- Handle questions and case escalations from Health Coaches , escalating to a higher level of clinical care as needed and per protocol.
- Maintains professional affiliations and enhances professional development to keep pace with the trends in diabetes and chronic care
Qualifications
Minimum Qualifications- Bachelor’s Degree in Nursing along with 3+ years of hands- on clinical experience delivering care.
- RN compact licensure. PA RN licensure required or ability to obtain PA licensure.
- Bilingual English/Spanish required.
- Strong computer skills and ability to use a variety of web app and information-dense productivity tools. Active experience using EMR systems.
- Strong familiarity with outpatient clinic workflows.
- Telemedicine experience is a plus.
- Startup mentality: ability to navigate in an evolving environment, balance competing priorities and manage several time-sensitive duties at once.
- Excellent organizational, interpersonal, and communication skills; ability to communicate effectively, both orally and in writing.
The US base salary range for this full-time position is $100,000 - $120,000 + bonus + equity + benefits. Our salary ranges are determined by role, level, and location. The range displayed on each job posting reflects the minimum and maximum target for new hire salaries for the position across all US locations. Within the range, individual pay is determined by work location and additional factors, including job-related skills, experience, and relevant education or training.
