Requirements
- Registered Nurse or Allied health Professional (Occupational Therapist, Physiotherapist, Dietician, Exercise Physiologist, Podiatrist and Psychologist) with relevant registration.
- Experience in care coordination and delivery of support services to people with complex and chronic care needs.
- Primary health care and care planning experience.
- Strong clinical knowledge and ability to translate care plans into practice.
- Strong understanding of chronic disease management practices, including workflows across the care team.
