Core-VA Solutions is seeking a detail-oriented and compassionate Case Management Assistant to provide essential remote support. This role plays a critical part in ensuring individuals receiving long-term care services have timely access to the resources, supports, and documentation they need. The ideal candidate is bilingual in Spanish and English, highly organized, and passionate about serving individuals with disabilities and their families. Experience in a health or human services setting is strongly preferred.
Key Responsibilities:
- Organize and maintain case files, records, service plans, and related documentation while ensuring compliance, confidentiality, and secure handling of sensitive information.
- Schedule and coordinate appointments, referrals, service authorizations, annual reviews, and follow-ups to ensure timely access to services and supports.
- Act as a liaison between clients, families, service providers, and community organizations — facilitating communication in both English and Spanish as needed.
- Assist in developing, monitoring, and updating individualized service plans, tracking deadlines, renewals, and progress to keep the case management team informed.
- Monitor compliance with state, and agency-specific regulations for service delivery and documentation standards.
- Collaborate with healthcare providers, therapists, and service coordinators to ensure clients receive holistic, person-centered care.
- Prepare and submit reports on client progress, service utilization, and outcomes to maintain compliance and transparency.
- Support case managers in resolving care-related concerns and addressing barriers to accessing required services.
- Communicate effectively with Spanish-speaking clients and families, providing interpretation and translation support to ensure equitable access to services.
- Provide feedback to improve workflows, service delivery processes, and overall client satisfaction.
Requirements
Experience:
- 2 years of proven experience as a Case Management Assistant or in a similar administrative role.
- Experience with scheduling, documentation, reporting, and coordination tasks in a healthcare or social services setting.
Skills & Other Requirements:
- Bilingual – Spanish and English required (written and verbal).
- Experience working with US-based clients is required.
- Strong organizational skills with meticulous attention to detail.
- Proficiency in case management software, Microsoft Office, or similar tools.
- Excellent interpersonal and communication skills with a client-first mindset.
- Ability to maintain strict confidentiality and handle sensitive information with professionalism.
- Ability to work both independently and collaboratively in a fast-paced environment.
