Overview
Chronic Care Staffing is seeking a compassionate and detail-oriented Remote RPM Coordinator – CMA to support patients living with chronic or high-risk conditions through Remote Patient Monitoring (RPM). In this role, you will monitor biometric data, educate patients on device usage, escalate critical findings, and act as a crucial liaison between patients and their healthcare providers. Your work helps improve patient outcomes, promote adherence to care plans, and reduce hospitalizations through real-time monitoring and timely intervention — all from a fully remote, HIPAA-compliant home office.
Responsibilities
- Conduct patient onboarding and device education calls for new RPM enrollees.
- Monitor daily biometric data (blood pressure, glucose, weight, oxygen saturation, etc.).
- Identify abnormal readings and follow escalation protocols to notify providers.
- Perform monthly RPM engagement calls and address patient concerns.
- Troubleshoot device connectivity or usage issues.
- Communicate with patients and caregivers about their conditions, goals, and progress.
- Support patient self-management and adherence to provider-developed care plans.
- Coordinate with healthcare teams to ensure timely responses to alerts.
- Document all interactions and interventions accurately in the EHR/RPM portal.
- Educate patients on lifestyle modifications, risk reduction, and monitoring importance.
- Comply with CMS RPM billing and documentation requirements.
- Meet productivity expectations for monitoring and outreach.
- Represent Chronic Care Staffing professionally and compassionately at all times.
Requirements
- Unencumbered active CMA or RMA certification.
- Active BLS certification.
- Ability to communicate clearly and compassionately via phone or messaging.
- Excellent documentation, time management, and critical thinking skills.
- Experience with EHR systems and cloud-based platforms.
- Familiarity with Google Suite.
Nice to Have (Optional)
- Strong understanding of Remote Patient Monitoring protocols and platforms.
- Proficiency in recognizing clinical red flags in biometric data.
