About Care Initiatives
Care Initiatives is Iowa's largest nonprofit operator of skilled nursing facilities, providing comprehensive senior care services since 1989. The organization serves more than 2,500 residents and patients through 43 skilled nursing locations, two assisted living facilities, seven independent living communities, and eight hospice offices. In 2024, Care Initiatives expanded beyond Iowa with a new hospice location in Omaha, Nebraska.
As a nonprofit organization, Care Initiatives reinvests millions of dollars into team member education, facility improvements, and new care programs. The organization has rebuilt 14 locations since 2000 and earned multiple National Quality Awards from the American Health Care Association.
