The Nurse Case Manager will support patients living with rare or disease or bleeding disorders to improve their health outcomes and quality of life by providing enrolment and reimbursement support to physicians and patients, and acting as a key resource to physicians who are prescribing this medication.

Requirements

  • Graduate of Registered Nursing Program or Registered/Licensed Practical Nursing program
  • Registered and in good standing with their provincial regulatory body
  • Minimum 2 years' experience in Patient Support Programs or equivalent
