The Population Health Pharmacy Team performs a variety of programs and services using population health management strategies and virtual communication to care for patients across the Advocate Health Midwest Region. This position will focus on chronic disease management using collaborative practice agreements to adjust and monitor medications for diabetes and chronic disease patients. It is an ever-evolving environment, where we engage patients in self-management of their chronic diseases and collaborate with other healthcare professionals to improve health outcomes.
The ambulatory pharmacist would be joining a team of pharmacists and pharmacy technicians with varying pharmacy backgrounds. Hours would be 8am to 5pm, Monday through Friday. This is a remote position but candidate must live in IL or WI and become dual-licensed within 90 days.
**Five or more years of broad diabetes management experience preferred**
MAJOR RESPONSIBILITIES
Develops competency in clinical practice including appropriate drug therapy for patients and prevention/resolution of drug related problems.
Develops competency to evaluate patients for drug-drug, drug-food, drug-lab, and drug-disease interactions. Individualizes drug therapy regimens including medication doses based on patient’s size, organ functions, and disease states.
Performs patient counseling, chronic disease management, comprehensive medication management and other patient care activities as directed.
Changes drug therapy as needed under a collaborative/scope of practice relationship with provider. Orders labs as needed for management of drug therapy and chronic disease management. Ensures all team members aware of unique patient situations. Pharmacists involved in the provision of services covered under a collaborative practice agreement will be required to demonstrate competence and achieve credentialing in the areas of chronic disease they are managing.
Supports department medication safety and quality improvement activities. Reports events through appropriate channels.
Supports clinical outcomes including improved CMS measures, decreased hospital admissions, chronic disease management clinical indicators, patient satisfaction, etc.
Participates in orientation/training of new team members. May work with interns, residents, and students to guide daily activities. May be preceptor-in-training.
MINIMUM EDUCATION AND EXPERIENCE REQUIRED
License/Registration/Certification Required: Pharmacist license issued by the state in which the teammate practices.
Education Required: Bachelor’s Degree in Pharmacy
