Izamar Hernandez

I’m a Licensed Practical Nurse and Charge Nurse with over 16 years of experience in patient care, staff management, and procedural compliance. I’m driven to maintain patient safety and comfort through therapeutic communication, strong documentation, and strict adherence to HIPAA guidelines and protocols.

In my current roles, I manage high patient loads while coordinating shift organization and planning, precepting and mentoring new staff, and delegating to CNAs and aides. I also provide skilled nursing care by recognizing changes in patient needs, implementing risk-reducing care, and educating clients and families on individualized routines.

I bring specialized experience across autism-behavioral health, memory care, dementia support, home health in-home care, and tracheostomy/ventilation-related support. I also strengthened my clinical foundation during pandemic relief as a Medical Assistant, focusing on infection control, PPE compliance, and safe delivery of care.
