In my roles, I review patient charts, care plans, and clinical documentation to support coordinated care delivery. I identify gaps tied to preventive screenings, chronic condition monitoring, and follow-up care, and I coordinate with providers, scheduling teams, and patients to maintain continuity.

I also lead quality and compliance programs across healthcare operations projects—conducting audits and documentation reviews, performing root cause analysis, and implementing process improvements. I develop quality scorecards and reporting frameworks, mentor teams, and use coaching and performance management to improve quality metrics.
