What I'm looking for

I'm looking to lead and support clinical teams delivering patient-centered long-term, sub-acute, or dementia care while improving quality outcomes, preventing rehospitalizations, and maintaining CMS and DOH compliance.

I manage daily clinical operations for a 60-bed long-term care unit with sub-acute overflow at Preferred Care at Wall, where I develop individualized care plans and implement interventions that reduce rehospitalizations.

I bring 15+ years of LPN experience across long-term care, sub-acute settings, assisted living, OB/GYN, and dementia care. As a Regional LPN Manager for Alzheimer’s & Dementia Care, I guided nursing teams across facilities, supported transitions of care, and strengthened quality and regulatory readiness.

I work closely with residents, families, physicians, rehabilitation, social services, and pharmacy teams to provide safe, patient-centered care. My work includes clinical assessments, medication and wound management, documentation audits, staff coaching, and compliance with NY/NJ DOH and CMS requirements.
