What I'm looking for

I'm seeking an RN Care Navigator or Case Manager role where I can use patient outreach, clinical assessment, education, care coordination, and relationship-building to support adults with acute and chronic conditions.

I've supported surgical and medical-surgical patients at Banner Health/Torrington Community Hospital, providing pre-admission teaching, postoperative telephone follow-up, comprehensive assessments, and coordinated transitions of care.

As an RN and former surgical technologist, I bring 12 years of combined perioperative experience, clear provider communication, patient education, clinical escalation, and accurate EHR documentation. I'm pursuing an RN Care Navigator/Case Manager role with Healthmap Solutions.
