I independently evaluate medically complex patients with acute and chronic conditions in post-acute rehabilitation and skilled nursing settings. I’m focused on improving mobility, function, pain control, and overall quality of life through comprehensive, patient-centered medical management.

I conduct new patient assessments, follow-up evaluations, and ongoing consultations across multiple facilities. I manage admissions and care transitions, coordinate interdisciplinary reassessments, and develop individualized treatment plans to support functional recovery while reducing avoidable hospitalizations.

I collaborate daily with nursing leadership, rehabilitation teams, attending physicians, specialty consultants, and case management. I review laboratory results, diagnostic imaging, hospital records, and therapy progress to guide clinical decision-making, and I communicate acute changes in condition with facility staff and providers.

I maintain timely, compliant electronic documentation in electronic medical records, using secure messaging, telephone communication, and virtual-centered care. I also perform procedures including trigger point injections and joint injections, and I ensure HIPAA compliance and accurate coding and billing for skilled nursing encounters.
