Adanze Nwokochah
@adanzenwokochah
Clinical pharmacist focused on medication safety, drug interactions, and patient-centered therapy optimization.
What I'm looking for
I’m a Doctor of Pharmacy candidate and clinical pharmacist-focused professional who partners across care settings to ensure safe, accurate medication therapy. In my current roles, I review medication orders and deliveries, confirm correct orders for diagnoses, and investigate drug-drug interactions to support better patient outcomes.
I’m especially drawn to medication safety and quality improvement. I complete monthly audits of MAR, orders, and clinical notes for compliance, perform EHR/medication-ordering system auditing, and use patient profiles to identify and mitigate drug-related problems, inappropriate dosing, and missing indications. I also educate patients and train staff on medication use, side effects, documentation, and informed consent.
My APPE experience spans internal medicine, pediatrics, critical care, transitional care, ambulatory care, community pharmacy practice, and medication therapy management, strengthening how I think and communicate clinically. Whether reconciling drugs administered or collaborating on alternative therapies, my focus stays consistent: reduce errors, improve medication decisions, and strengthen team readiness through education.
