Role Overview
We are looking for a compassionate and driven Patient Navigator to join our team. Required Qualifications
- 3+ years of experience in healthcare or a related field, with a focus on patient advocacy or navigation.
- Strong understanding of healthcare systems, insurance processes, and community resources.
- Excellent interpersonal and communication skills, with the ability to build rapport with patients and their families.
- Demonstrated ability to work independently and manage multiple patient cases simultaneously.
- Proficient in using electronic health records (EHR) and patient management systems.
Preferred Qualifications
- Experience working with diverse patient populations and understanding cultural competency in healthcare.
- Certification in patient navigation or a related field.
- Knowledge of chronic disease management and health literacy principles.
