About us

Sue Ryder Leckhampton Court Hospice in Cheltenham is Gloucestershire’s only inpatient unit for specialist palliative care. We support people aged 18 and over who are living with life-limiting conditions such as cancer, heart failure and lung disease by combining specialist medical care for managing pain and other symptoms with other emotional, practical and spiritual support. We are able to provide care on our inpatient unit, virtually and in person though our community hub and in people’s home though our Hospice at Home service.

About the role:

To provide clinical and operational leadership and management of community-based clinical and wellbeing services, ensuring the safe, effective and compassionate delivery of care to people and their families in the community and in-reach to the Inpatient Unit as appropriate.

The post holder is responsible for delivery of community services within clinical governance frameworks set by the Head of Clinical Services, working collaboratively with inpatient services to ensure continuity of care across pathways. Working together with the Head of Clinical Services, the role holder will be responsible for ongoing review of service performance and managing progress against agreed outcomes; building strong relationships within the local clinical teams and across the wider healthcare system to maximise impact of the services; and developing, implementing and reviewing ways of working in line with population and business need.

Clinical and operational leadership and management:

  • Day to day management of integrated community team (which may include CNS, Hospice at Home, wellbeing and community support and therapy teams)

Clinical Quality and Governance:

  • Ensure services are delivered in line with agreed care standards, policies and procedures;
  • Implement audit activity and quality improvement actions agreed by HoCS

Integrated care development:

  • Work collaboratively with Ward Manager and inpatient teams to ensure integrated care;
  • Represent community services in local operational forums as required;
  • Work with Primary Care Networks (PCNs) and other external stakeholders to deliver integrated service models, fostering positive collaboration

About you:

Essential

  • Minimum Criteria Registered Nurse
  • Relevant degree or equivalent experience
  • Supervision qualification or relevant experience
  • Evidence of leadership and management development
  • Evidence of continued professional development
  • Significant experience leading community or out-of-hospital clinical services

Desirable Criteria

  • Evidence of operational leadership and people management skills (including volunteers)
  • Experience implementing service change within agreed governance frameworks
  • Understanding of palliative and end-of-life care in community settings (including a knowledge and experience of Advanced Care Planning)
  • Experience working across organisational boundaries
  • Excellent communication and relationship-building skills
  • Knowledge of health systems in England, including the NHS 10 Year Plan
  • Proven knowledge of developing and implementing effective services, working alongside partner organisations, particularly across community teams.
  • Ability to work both autonomously and with others, demonstrate initiative and possess advanced communication, good attention to detail and strong organisational skills
  • Experience of working with diverse communities and building engagement to reduce health inequalities
