About the Role:
At Plume, we envision a member experience that provides thoughtful, expert, timely, and gender-affirming care that celebrates and enhances the quality of life for every trans person. As a Care Coordinator, you are the patient's first point of contact. You ensure excellent patient care and satisfaction within a fully remote, telehealth environment, reporting directly to our Care Coordinator Manager. This work is profoundly personal and heart-driven, so we want teammates who, above all else, care. Responsibilities:
- Answer current and prospective patient questions.
- Triage inquiries to the appropriate medical staff.
- Ensure accurate data flow.
- Help support our members' labs, pharmacy, and other resource needs.
- As we grow and continue to expand our services, aspects of member care may encompass facilitating peer support groups and supporting care management for members.
Must-Haves:
- A strong understanding and appreciation for the trans experience and a desire to increase access to gender-affirming care.
- 2+ years in a customer service role with a demonstrated history of excellent customer/patient/member service.
- Great attention to detail in addition to data management and data entry skills.
- Respectful and empathetic written and verbal communication.
- Basic computer skills and comfort with learning new computer systems.
- Comfortable interfacing with patients and colleagues within a fully remote environment.
Compensation:
- $55,000 Annual Salary
