We are currently seeking a Generalist Assessorto join our Clinical Department. This is a meaningful opportunity for professionals who are passionate about helping others live healthier, more independent lives. As a Generalist Assessor, you’ll work directly with individuals to assess their needs, develop personalized care plans, and connect them with essential services and supports. Key Responsibilities

  • Adheres to the CareStar Rule in performance of job responsibilities.

  • Understands and complies with CareStar Policies and Procedures.

  • Maintains confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offence.

  • Follows the Acceptable Use Policy while using any information systems owned or controlled by CareStar.

  • Complete remote InterRAI Assessments for Iowa and CANS Assessments for Ohio.

  • Perform initial and ongoing assessments using approved tools and procedures.

  • Evaluate functional status, service needs, supports, and risks.

  • Complete all required documentation accurately and on time.

  • Communicate assessment findings and follow-up needs to designated parties.

  • Coordinate with members, caregivers, case managers, MCOs, and agency staff as needed.

  • Maintain compliance with policies, confidentiality rules, and reporting expectations.

  • Participate in training, quality assurance, and calibration activities.

    Minimum Qualifications

  • Bachelor’s degree in a health or human services field preferred.

  • Experience in assessment, care coordination, social services, or related work preferred.

  • Effective organizational, interpersonal, communication and leadership skills.

  • Effective computer skills with familiarity with suite of Microsoft Office programs.

  • Valid driver’s license and car insurance as required by State law.
