About the Role
We are seeking a Licensed Care Management Clinical Support Specialist to support a U.S.-based clinical care management team.
This role is ideal for a licensed healthcare professional with experience in patient coordination, clinical documentation, and regulatory compliance who can confidently support care management operations in a remote setting.
Active U.S. Care Management licensure is mandatory for this role.
Key Responsibilities
Clinical & Operational Support
Provide remote administrative and clinical support to ensure smooth daily operations
Manage client intake and onboarding processes
Schedule clinicians for home visits using EMR/EHR systems
Maintain accurate, timely, and compliant medical documentation
Patient Care Coordination
Communicate with patients, families, and caregivers via phone, email, and secure platforms
Respond to patient inquiries within scope of practice (no diagnosing or prescribing)
Assist in the development and documentation of care plans under licensed supervision
Coordinate follow-ups, referrals, and continuity of care
Compliance, Billing & Documentation
Ensure full compliance with HIPAA, company policies, and U.S. healthcare regulations
Support insurance billing documentation and care management reporting
Generate invoices, coordinate with vendors, and track required documentation
Maintain strict confidentiality and patient-centered standards
Technology & AI-Supported Workflow Optimization
Utilize EMR/EHR systems efficiently for documentation and scheduling
Leverage approved AI and digital tools to assist with documentation structuring, workflow efficiency, and reporting (AI supports operational efficiency only — not clinical decision-making)
Identify opportunities to improve documentation accuracy and process efficiency
Required Qualifications
Active U.S. Care Management license (mandatory)
Minimum 1+ year of recent healthcare or care management experience (home health or care management preferred)
Strong working knowledge of EMR/EHR systems
Minimum English proficiency: B2+ (professional written and verbal communication required)
High attention to detail and strong organizational skills
Ability to work independently in a structured remote environment
Availability during U.S. business hours
Preferred Qualifications
CNA, MA, RN, LPN, or Home Health Aide certification
Experience with insurance billing or care coordination
Experience supporting home health, hospice, or care management programs
Familiarity with telehealth or remote patient coordination
Experience using AI-supported documentation tools
Requirements
High school diploma or equivalent (healthcare or medical office training preferred)

Ability to work flexible hours and participate in on-call rotations if required
