Job Description:
A Form Designer is expected to provide basic electronic form design and form QA for assigned Form Design projects.
You will:
Create electronic forms based upon direction from Senior Staff and submit completed files to Senior Staff for review.
• Design electronic forms with the correct interactive elements, proper margins, correct grammar, and verbiage in an easily readable format.
• QA assignments before turning over to Senior Staff to ensure accuracy of verbiage.
• Follow best Formatter design practices provided by Senior Staff.
• Must meet deadlines given for form files to be returned to Senior Staff.
• Must communicate with the Senior Staff with any questions or concerns during the design project. Communicate if there will be any delays in the project.
• Form templates will be provided for each project. The Form Designer will need to use the template to create form files in Formatta based on the originals provided by the client.
• Be able to use Microsoft Word/Adobe Acrobat Pro to view client originals and use these Attention to detail is a must.

You have:
• Minimum: Bachelor’s Degree in Computer Science or related field plus specialized business or industry training, including proficiency in MS PC and Server products.
• Appropriate experience/training may be substituted for formal education at the discretion of the manager or the Executive Team.
• Intermediate level Microsoft Windows, MS/Office and Adobe Acrobat
• Intermediate level knowledge of a form design application
• Ability to develop and document procedures
• Excellent oral/written communication skills
• Analytical skills
Certification and Licensure: Prefer certification or licensure as dictated by the functional
area to be supported.
Working Hours: 03:30pm-12:30am
