This position supports patients enrolled in chronic care management (CCM) and remote patient monitoring (RPM) programs. The ideal candidate will provide high‑quality patient care, education, and coordination, ensuring adherence to care plans while collaborating with multidisciplinary teams.Key ResponsibilitiesConduct virtual outreach with patients to review care plans and provide educationMonitor patient health status via RPM tools and elevate clinical concerns appropriatelyDocument all interactions in compliance with CMS, payer, and internal quality standardsCoordinate post‑discharge follow‑up and specialist appointmentsCollaborate with nurses, pharmacists, and other care team members to optimize outcomesFoster patient engagement, adherence, and trust through empathetic communicationRequired CredentialsActive Nevada LPN or LVN license in good standingMinimum of 2 years of clinical nursing experience (telehealth or chronic care preferred)Strong verbal and written communication skills in EnglishCompetent in patient documentation and clinical judgmentExperience with CCM and/or RPM workflows is a plusAbility to work independently in a HIPAA‑compliant, remote environment#J-18808-Ljbffr
