Job Summary:
As a Care Advisor, you will serve as the primary point of contact for prospective clients and their families. Your role will be twofold:
- Marketing & Outreach: Execute our established marketing strategy to build relationships with referral sources, healthcare providers, and community organizations.
- Intake & Care Planning: Meet with families to assess the needs of elderly clients, develop personalized care plans, and coordinate the start of care to ensure a seamless transition.
Key Responsibilities:
- Client Engagement & Outreach:
- Proactively follow our proven marketing process to generate leads and nurture relationships with referral partners.
- Represent the company at community events, health fairs, and networking opportunities.
- Intake & Needs Assessment
- Serve as the initial point of contact for families, providing compassionate guidance and clear information about our services.
- Conduct in-home or virtual assessments to determine clients' care needs and preferences.
- Care Plan Development:
- Collaborate with families and internal care teams to design customized care plans that promote safety, dignity, and enhanced quality of life for seniors
- Process Management & Documentation
- Follow established protocols for intake, marketing, and care planning while ensuring accurate documentation and timely follow-ups.
- Communicate regularly with team members and referral sources to keep everyone informed and aligned.

- Valid driver's license and reliable transportation (if required for in-home assessments).
