General Description The Program Manager – Multifaceted Populations is responsible for collaborating with internal and external stakeholders to develop, oversee, and enhance Enhanced Care Management (ECM) and Community Supports programs for HPSM’s CalAIM members. They provide operational and relationship support for contracted providers and vendors, focusing on ensuring quality, accessibility, sustainability, and integration. This role emphasizes supporting ECM providers, including but not limited to those serving HPSM’s justice-involved member population. Qualifications The following represents the typical way to achieve the necessary skills, knowledge, and ability to qualify for this position: Education and Experience
- Bachelor’s degree in business administration, public health, health care management, or public policy required and five (5) to seven (7) years of experience as a program/project manager.
- Master’s Degree (MBA, MPH, or MPP) preferred.
- Experience with CalAIM, correctional health, and justice-involved populations preferred.

- Advanced verbal and written communication skills, with the ability to engage clearly and effectively across audiences and organizational levels.
- Advanced quality improvement/process improvement techniques skills.

Duties & Responsibilities Essential Functions Program Management
- Manage programs from initiation through delivery and monitor for continuous improvement.
- Plan, execute, and evaluate program initiatives to improve performance, delivering results to a wide range of audiences.
- Apply an iterative approach to process development and implementation, continuously refining strategies based on feedback and outcomes.
- Utilize critical thinking and creative problem-solving skills to proactively respond to dynamic and rapidly evolving program landscapes.
- Continuously align program goals with departmental and enterprise-wide objectives.
Partner Engagement
- Function as the main point of contact with external partners and internal stakeholders to support collaboration and program delivery.
- Lead onboarding and serve as primary liaison with external partners and vendors.
- Educate and work with external partners to identify innovations that improve outcomes.
- Participate in cross-functional business planning to support initiatives focused on member care and compliance.
- Work interdepartmentally to support CalAIM and related operational activities.
CalAIM Program Operations
- Function as an organizational subject matter expert on CalAIM Enhanced Care Management and Community Supports operations, regulatory environment, and financial structure.
- Utilize key performance indicator dashboards to assess and communicate program impact.
- Support development and implementation of process improvements and best practices by leading collaborative workgroups.
- Align stakeholders to deliverables, help contributors be accountable for deliverables, and escalate concerns as needed.
Secondary Functions
