About the Role: Activa Home Health is expanding its Hospital at Home program, delivering hospital-level care to patients in their homes as an alternative to inpatient hospitalization. Patients remain formally admitted to a hospital partner while receiving coordinated in-home care supported by physician oversight and continuous clinical monitoring.

Position Summary: Provide hospital-level skilled nursing care to patients enrolled in a Hospital at Home program. RNs deliver high-acuity care in the home while completing timely, accurate clinical documentation in Kinnser (WellSky).

Key Responsibilities:

• Deliver hospital-level clinical care in the patient's home
• Complete timely, accurate, and thorough clinical documentation in Kinnser (WellSky)
• Communicate patient status changes per established clinical protocols
• Adhere to program workflows, response expectations, and professional standards
• Provide care that reflects hospital-level quality and accountability

Required Qualifications:

• Active, unrestricted professional license
• Strong assessment, clinical judgment, and documentation skills
• Ability to work independently while meeting hospital-level expectations
• Comfort with mobile technology and electronic documentation systems
• Reliable transportation and strong attendance record

Preferred Experience:

• Acute care, hospital, ICU, ER, or step-down experience
• High-acuity home health or complex medical care experience
• Experience collaborating with interdisciplinary clinical teams
