At Axial Health Consultant PVT LTD, I conduct second-level clinical chart reviews to ensure documentation reflects patients’ severity of illness, risk of mortality, and overall clinical picture.
I identify documentation clarification opportunities and write and validate compliant queries using ACDIS best practices and AHIMA guidelines. I also help reconcile code and DRG assignment discrepancies between CDI and Coding teams.
Previously, at Medusind Solutions, I worked on medical records, coding, and claims for Medicaid and Medicare. Earlier, at Rotary Physiotherapy Centre, I supported patient assessments and individualized treatment plans under senior physiotherapists’ supervision.
