The PRN Clinical Intake Coordinator serves as a liaison between clinical operations and referral sources to support timely, appropriate patient care. This role helps build and maintain community relationships, provides education about hospice services, and supports patients, families, physicians, case managers, discharge planners, social workers, and other healthcare partners throughout the intake and referral process.

Requirements

  • Associate degree in Nursing
  • Current Alabama driver's license
  • Current automobile liability insurance
  • Current nursing license
  • Reliable transportation
  • Cell phone
  • Knowledge of Medicare and other payer guidelines related to hospice services
  • Strong communication, relationship-building, and public education skills
  • Ability to identify problems, develop solutions, and support strategic planning
  • Working knowledge of sales principles and community outreach
  • Understanding of geriatric community needs and referral patterns
  • Knowledge of physician, hospital, skilled nursing facility, case management, and discharge planning needs
  • Ability to communicate hospice services effectively across all levels of the healthcare industry
  • Ability to manage multiple priorities, meet deadlines, and adapt quickly to changing needs
  • Commitment to teamwork, professionalism, and confidentiality
