Job Title: Clinical Program Consultant, Virtual Care Management - VCM

Hourly Pay Range: $38 - $45 per hour
Location: Remote

Job Type: Part-Time, Independent Contractor

Schedule: Monday - Friday, 20 - 29 hours per week. Schedules are subject to change based on operational needs.

In addition, Cardiac RMS by DocGo is a provider of Virtual Care Management Services using a proven platform that connects patients, providers, and the broader healthcare team. Cardiac RMS by DocGo utilizes industry-leading, cloud-based software systems for the provision of services. The management and clinical team members have years of experience in the industry and maintain the highest levels of certifications and expertise.

Position Overview:

The Clinical ProgramConsultant, Virtual Care Management (VCM), will serve as the organization’s subject matter expert in virtual care program design, implementation, and optimization. This roleis responsible foradvancing best practices, workflows, and performance measurement strategies across key virtual care programs, including Principal Care Management (PCM), Chronic Care Management (CCM), Transitional Care Management (TCM), and Remote Patient Monitoring (RPM).

This individual will play a critical role in ensuring program excellence, scalability, compliance, and measurable outcomes. The Advisor will partner closely with VCM leadership to mentor and support management and staff while driving continuous improvement across all virtual care services.

Responsibilities:

Program Development & Optimization

  • Lead the design, refinement, and scaling of virtual care management programs (PCM, CCM, TCM, RPM)
  • Establishand standardize best practices, workflows, and operating procedures
  • Ensure programs align with regulatory requirements, payer guidelines, and industry standards
  • Identifyopportunities for innovation and continuous improvement across all VCM services

Performance Measurement & Effectiveness

  • Define key performance indicators (KPIs) and success metrics for all VCM programs
  • Develop frameworks for measuring clinical outcomes, patient engagement, and operational efficiency
  • Analyze program data to drive performance improvements and inform strategic decisions
  • Partner with leadership to report on program effectiveness and ROI

Subject Matter Expertise

  • Serve as the internal expert on virtual care management programs, including billing, compliance, and operational best practices
  • Stay current with industry trends, CMS guidelines, and evolving care models
  • Provide guidance on program expansion, payer strategies, and service offerings

Training & Mentorship

  • Train and mentor VCM management and frontline staff on program protocols and best practices
  • Support onboarding and ongoing education initiatives
  • Promote a culture of excellence, accountability, and continuous learning

Qualifications:

Required:

  • Significant experience in virtual care management programs (PCM, CCM, TCM, RPM)
  • Demonstrated success in program development, implementation, and optimization
  • Strong understanding of CMS guidelines, billing requirements, and compliance considerations
  • Experience developing workflows, SOPs, and performance metrics
  • Proven ability to analyze data and drive operational improvements
  • Experience training, mentoring, or advising care management teams

Preferred:

  • Clinical background (RN, NP, or similar) or equivalent healthcare leadership experience
  • Experience in value-based care models
  • Familiarity with digital health platforms and remote care technologies
